Two Rivers High School, Friendly Hills & Heritage Middle Schools are required to follow the Minnesota State High School League (MSHSL) rules that require sports physicals for all MSHSL sanctioned sports. Two Rivers High School also requires sports physicals be on file for: Summer Strength & Speed, Offseason Strength & Speed, Pom Squad, Bowling, Winterguard, Cheer Team, Drumline, Marching Band, Flag Football

The sports physical must (1) clear the student for participation in sports, (2) be dated within three years of participation, and (3) remain current throughout their sport/activity season. Without this up-to-date clearance physical, you will not be able to register your student. Check your child’s profile for current physical date on file. 

**Please note: Well child summaries often do not clear your child for sports, so be sure to ask that a sports physical be completed**

Completed sports physicals can be submitted in 3 ways:

(1) Fax to 651-403-7271, Attn: Activities
(2) Scan & Email form to dustin.boll@isd197.org
(3) Drop off signed form at Activities Office
